Thank you for your interest in Gabe's Chemo Duck program. It is our mission to provide education and comfort to children living with cancer through therapeutic tools and play. By agreeing to be a part of our hospital program, you are committing your organization to making a difference in the lives of the children for whom you care. The Gabe's Chemo Duck Hospital Program is an opportunity for hospitals to provide each pediatric cancer patient with a soft, cuddly friend to assist them in understanding the changes in their lives and how to cope with what they are going through.

Here are some commonly-asked questions regarding our hospital program. We hope to not only provide answers, but also encourage you to sign up for Gabe's Chemo Duck Hospital Program. We feel confident that you will immediately see the positive impact this program has on your patients.

THE PROGRAM

What materials are included in the program?
When a hospital joins Gabe's Chemo Duck program they receive the following:
. One training binder with useful information about Gabe's Chemo Duck Program, information on effective distribution to families and parts of Gabe's My Heart's Parent to Parent resources
. Three copies of the Chemo Duck DVD that shows parents how to use Chemo Duck
. Both Port and Hickman Brovaic Ducks are available; each Duck is accompanied by an introductory book,
"Meet Chemo Duck"
. Copies of the"I'm Still Me" activity book. This thought-provoking and educational workbook is designed to address topics relevant to the cancer diagnosis. The activities are designed to appeal to children of all ages.
. Access to our printable materials including the "Parent to Parent Guide", a book developed from the experiences of many families who have faced childhood cancer head on. "Parent to Parent" offers knowledge and insight into creating a pathway through childhood cancer.

How do we manage the program?
Typically, hospitals that offer Chemo Ducks to their patients do so through their Child Life Department. Because child life specialists are so closely connected to the patients and their families, it makes sense for them to coordinate the most appropriate time to present their patients with their Chemo Duck. The Gabe's Chemo Duck team members are happy to assist you in setting up the program and can partner you with other peer organizations to help discuss their experiences with the program.

How do I sign up and order?
To sign on as an official Chemo Duck hospital, you simply complete the hospital sign up form on our Web site, www.chemoduck.org. A representative from our office will contact you and set up a time to consult with you about your desire to be a part of Gabe's Chemo Duck program. Once everything is set up, your first order will be processed. Currently Gabe's My Heart is unable to carry a stock of Chemo Ducks and our orders are placed twice per year. After Gabe's My Heart's order is placed shipments usually take approximately 12 to 15 weeks. We are working hard to rectify the long delays in ordering and receiving the Chemo Ducks and appreciate your patience in the interim.
